By enrolling in any current or future programs of 4-Skills Soccer Academy (NL), the participant and their parent or guardian acknowledge and agree to the following terms:
Participation & Assumption of Risk
Participation in 4-Skills Soccer Academy (NL) activities, including the use of facilities, is done at the participant’s own risk. 4-Skills Soccer Academy (NL), including its owners, staff, volunteers, and agents, shall not be held liable for any injuries, illnesses, or property loss sustained by the participant or their family, whether occurring during or related to participation in any of our programs or while on or around the premises.
The participant and their parent/guardian accept full responsibility for any injuries or damages and, by signing below, agree to release, discharge, and hold harmless 4-Skills Soccer Academy (NL) and all associated parties from any and all present or future claims, demands, damages, liabilities, or rights of action, whether arising from ordinary negligence or otherwise.
Code of Conduct
Participants agree to follow the rules of conduct and fair play as established by 4-Skills Soccer Academy (NL). Failure to comply may result in temporary or permanent suspension from the program, at the discretion of the staff.
Medical Authorization
In the event of an accident or illness where the parent or guardian is not present, the undersigned authorizes 4-Skills Soccer Academy (NL) staff to make decisions regarding necessary medical treatment or hospital care.
Use of Photos & Media
By enrolling, the participant and their parent/guardian grant permission to 4-Skills Soccer Academy (NL) to use any photographs, video footage, or likenesses of the participant for promotional purposes, including but not limited to brochures, social media, and team media.
Health Safety Waiver
The participant and parent/guardian voluntarily accept and assume all risks associated with illness (including COVID-19), personal injury, disability, or death that may arise from participation in 4-Skills Soccer Academy (NL) programs. The participant agrees to release and hold harmless 4-Skills Soccer Academy (NL) and its representatives from any and all related claims or expenses.
Payment & Refund Policy
By enrolling, you agree to the terms and policies of 4-Skills Soccer Academy (NL). All transactions are final. Refunds, if issued, will be granted as credits at the discretion of the academy.
